How do I complete the state verification report process?

Thank you for contacting the NCCAOM. To request exam results to be forwarded directly to the state of choice, please login to your NCCAOM online portal. Once navigated to your portal home page, please select "Request State Verification" as shown below:
 

 
Once selected, please proceed through the process to have your results sent to your desired state. Processing time for state verification results is 7 business days. The results will be forwarded to your desired state electronically and will include your certification date and expiration date (if you are certified), your CNT date only (if the state requires your actual CNT certificate, please contact CCAHM), and all exams pass/fail. Be advised that all requirements required by your state must be completed prior to submitting the request. If all requirements are not met upon request submission, a new request would be required.
